All of this information is obtained directly from you before your consultation. The data is filled out in an intake form. This information is reviewed and added to during the consultation. Care notes are taken during your appointment, and remedies are documented. If you require a copy of this information this service is subject to a reasonable administration fee.

Why do I need Consent?

Your Consent is required before our meeting, and by booking an appointment on my online scheduling service, you will be asked if you consent to the Terms & Conditions.
